Hello faculty I saw this patient today because she felt the implant crown was loose. Indeed the screw has come loose. I removed the crown and rechecked ISQ (80/81). I retighten the crown back on and torqued to 30N/cm2. When I took the PA I was dismayed to see bone loss on both the mesial and distal of the fixture. The crown is about 17 months old. Dr. Kwon, When you make a surgical guide for single tooth implants, it seems from you lectures that there is a wax up done and then a slot through the center of the matrix centered through the tooth to accomodate 1.5mm twist drill. Is this correct? Can you comment on type of material or instructions to lab for fabrication of pilot guide stent. Thanks Chad Denomme...

Hello I have a case where pt is looking for a 35 36 implant option. 36 was extracted 3 month ago. CBCT was taken after 3 month. 36 was socket grafted with prf/allograft/collagen membrane. 3 was extraced years ago. I believe there is insufficient bone to place 35 and 36 in ideal position. Hello I did an extraction with allograft and PRF and collagen membrane in june 2016. (post op exo attached) I took the CT scan 3 month later Sep 2016. I am not sure if this patient is ready for an implant yet. the graft area does not seem to be as dense as the surounding bone (270 vs 700+). Dr. Kwon, Can you go over the implant removal protocol for nobel replace select 5.0 or wide platform. I made a few notes at the course and have a slide showing some numbers. Seeing this is my first removal can you walk me through the use of the hiossen fixture removal kit to have the best chance at getting this implant out.
