I am planning to place 45,46 implants both 5x8.5.My question is regarding grafting as in the plan both implants will require grafting.Obviously would utilise sticky bone to cover threads.Due to proximity of mental nerve to the 45 and the location can someone comment here on there thoughts as far as flap design and release. I would think placing cover screws and trying to achieve primary closure is my goal.Chad...

This is in response to the latest reply regarding SA4 Lateral Sinus Augmenation.Due to the combination of issues I am trying to overcome here I think I will refer this case.If my GBR skills were at a higher level I think I would do GBR first and try to improve vertical and horizontal dimensions of the bone.Doing both at the same time would increase my chances of having complications that could effect eachother. Hi Everyone,I am planning this case for lateral sinus augmentation.The patient desires to replace her 25 and 26.Patient is discontinuing Plavix 3 days prior to surgery. Good morning everyone,I have done a few of these cases but not many.Any advice on how to get the best out come? There is about 2mm of bone and I am thinking augmentation first, allow to heal and then implant 6 month later. Would be nice to engage an implant and use CASK but I don't think 2mm of bone is enough. I performed lateral sinus augmentation 9 months ago on the right side. No post op issues.A new CT was obtained before placing the implants 15 and 16 and there was plenty of bone.I placed 15 and 16 in September and did a 10 day post op check and removed sutures at this time. Mark, This patient requires single tooth implant 16. I have 4-4.5 mm of vertical bone to the sinus.The OMC is patent. The patient suffers from season allergies only (sinusitis) but no history of chronic sinus infection.Can you review the radiographs attached, CT is very recent. Hi guys,I'm going to be doing my first All on 4 case so I decided to use nSequence to give me a bone reduction guide as well as an implant placement guide. Turns out, the case also has some tori that the technician believes that is not removed will either make the guide very difficult to place as it's thickness will be encroaching on the tongue space. Helloi just placed an 4.0x11.5 on Saturday in the 15 position. Soft bone. Type 3/type 4. Not sure. Osteotomy underprepared to 3.5 but I still managed to get a spinner. I put a cover screw and closed the flap. But now, one day later, I wish I had put a 4.5x11.5 . At first I was worried about lack of buccal bone but now I think it can accommodate a 4.5.
